I, the undersigned parent/legal guardian, hereby give my permission
for my child to participate in the Soul Sundays Kids Event. I acknowledge that participation is voluntary and that children must be accompanied
by a parent/legal guardian or an authorized adult
representative designated by me.
I understand that Soul Sundays, its staff, volunteers, and event
partners will take reasonable precautions to provide a safe
and welcoming environment. However, I acknowledge that participation
in activities may involve inherent risks and agree to take responsibility
for ensuring that my child follows event rules and safety instructions.
In case of an emergency, I authorize Soul Sundays staff to
contact me and, if necessary, seek appropriate medical assistance
for my child if I cannot be immediately reached. Any medical expenses incurred will remain the responsibility of the parent/legal guardian.
